Sign Up Forms

 Each family will need to fill out/sign 1 Sign up Sheet (please pick the form based on the number of dancers you have in your family) and a Code of Conduct.

You can either sign up in person, at our open house, or online! If you choose to sign up online, we will have your welcome packet ready to go for you on your first day!  This will include your calendar, remind app signup, basic information and code of conduct.

Please remember there will be a concert at the end of the dance season (in June) and ALL DANCERS will be included.  If this will be a problem, please contact the studio before signing up.
